While I no longer am a provider with insurance companies, I will provide a 'Superbill' at the end of each month for those clients who wish to bill their insurance companies for out-of-network providers. Please note that services may be covered in part by your insurance carrier for out-of-network providers. You can check your coverage by asking the following questions:
